From time to time, MACO undertakes to develop special projects that are not yet ready for program funding or that involves a large number of external stakeholders.
What is "patient navigation"?
Patient navigation is used to denote a system or professional
role that is intended primarily to expedite patient access to
services and resources, and to improve the continuity and coordination
of care throughout the cancer continuum. Synonyms used in North
America include "case manager," "clinical coordinator,"
"cancer support nurse," and "follow-up nurse."
Over the past few years, patient navigation for cancer care has sparked the attention of health care planners in Canada. This approach has been proposed as a potential solution to the sometimes fragmented care received by cancer patients and their families. Patient navigation projects are currently evolving nationwide, in British Columbia, Nova Scotia, Quebec and Yellowknife. Evaluation data from Nova Scotia indicates that patient navigation contributes to overall improvements in the cancer care system itself by addressing problems related to integration, coordination and continuity of care.
ACB wants to explore this approach for its cancer population
and stakeholders in the province and institutional leaders share
this interest. A Patient Navigation Advisory Committee was formed
in the fall of 2004 to begin planning a needs assessment for Alberta.
